Single adult memberships start at $75 per month for adults 60 and younger, and $80 per month for adults over 60. Family plans start at $170 per month for two adults and up to two children. Pediatric plans run $85 per month for children under age two and $70 per month for children two and older. Business plans are available at special rates.

The membership covers unlimited office visits, an annual physical, same-day and next-day scheduling, and 24/7 direct physician access by phone, text, and email. It also includes joint injections, breathing treatments, cryotherapy, lesion removal, laceration repair, IV fluids, sports physicals, custom weight loss plans, holistic treatments, and limited acupuncture sessions at no additional cost.

Black Bag Family Healthcare operates on a direct primary care model and does not bill insurance for membership fees or included services. Many members pair their membership with a high-deductible health plan or health sharing plan for coverage of hospitalizations or specialist care.

Yes. The practice offers same-day and next-day scheduling. Each provider limits their panel to around 700 patients, so the team can consistently prioritize prompt access for members.

Yes. Members can contact their physician by phone, text, or email 24 hours a day, seven days a week, including weekends and holidays. There is no additional charge for after-hours communication.

Yes. A physician can come to your home or office for acute appointments within the service area. An additional fee applies for house call visits.

Yes. Members can connect with the medical team via webcam, email, text, and other digital channels. Telemedicine access is part of the standard membership.

Black Bag passes wholesale pricing directly to members for labs and medications. Members pay the same rate the practice pays, which typically results in significant savings.

Yes. Memberships are based on a yearly contract. The main exception is if a member moves outside the practice's service area, in which case the contract ends.
