Elite Concierge includes unlimited in-person and virtual visits, direct text, email, and phone access to Dr. Bazzaz, evening and weekend care, same or next-day appointments, hospitalization coordination, and free initial hormone and weight management consultations.

BazzazMD does not bill insurance for membership fees. The practice operates on a direct membership model with no copays or deductibles. Members may still carry insurance for hospitalizations, specialist referrals, or services outside the membership scope.
