Avalon Primary Care

Monthly membership at Avalon Primary Care costs $70 per person for adults ages 26 to 64. Children and young adults up to age 25 pay $40 per month, or $20 with a parent or sibling member. Family rates are also available. Medicare recipients pay no monthly fee. Fees are payable by cash, check, credit or debit card, and HSA accounts.

The membership fee covers all office visits, including physicals, well child exams, sick visits, counseling visits, and paperwork visits such as FMLA or disability forms. It also includes most procedures, such as skin lesion removals, joint injections, allergy shots, sutures, and splinting. Osteopathic Manipulative Therapy is included for members. Members also receive significant discounts on medications and lab testing.

Avalon Primary Care does not accept insurance other than Medicare and its supplements. Dr. Butler bills Medicare and most Medicare supplemental and advantage plans directly. For all other patients, the practice operates on a direct-pay membership model. Dr. Butler strongly encourages all patients to maintain their own health insurance for services outside the practice.

Appointments at Avalon Primary Care run 30 to 60 minutes. That time allows Dr. Butler to listen thoroughly, review your history, and work through your concerns without rushing. Same-day appointments are available, and Dr. Butler offers telemedicine visits by phone, email, text, or video call.

Yes, Dr. Butler offers both telemedicine and home visits. Patients can reach her by phone, text, email, Skype, or FaceTime. Home visits are available for established patients at a fee of $75 and take place during daylight hours. Weekend care is also available if Dr. Butler comes into the office, at a fee of $50 for all ages.

Avalon Primary Care performs CDL physicals and Professional SCUBA Diving physicals. Members pay $85 for these, and non-members pay $110. The practice also performs sports physicals and driver's physicals as part of the standard membership services.

New patients can sign up directly through the Avalon Primary Care enrollment portal online. A one-time consultation fee applies before the monthly membership begins. That fee is $30 for patients up to age 25 and $70 for all others. The practice currently accepts new patients.

Yes. Members receive significant discounts on common medications and lab testing, including blood work. Dr. Butler also helps patients find the best available rates for x-rays and other imaging studies. If the practice cannot provide a particular service, Dr. Butler works to find the most cost-effective outside option.

Dr. Butler chose the name Avalon from three sources meaningful to her. She grew up near Santa Catalina Island in Southern California, where the city of Avalon holds cherished memories. She also spent time near Avalon, New Jersey. The name connects to the mythical island of Avalon from the Legend of King Arthur, described as a healing island filled with fruit trees. The practice logo represents those trees, the chalice, and the sword of Excalibur.
