Aevitas membership starts at $650 per month for a single person. The practice offers three tiers: Core, which requires benchmarking and provides access on an as-needed basis; Clarity, which includes three annual visits, direct access, and comprehensive testing; and Legacy, which includes unlimited visits, full testing coverage, priority access, and complete primary care plus longevity medicine. Special pricing may apply for spouses, dependents, and family members. HSA funds are eligible for use toward membership.

Membership includes 60-minute appointments, same-day availability, telemedicine access, and direct communication with Dr. Valice by text, phone, or email. Depending on the tier, members also receive comprehensive lab testing, hormone evaluations, advanced lipid panels, body composition tracking, and access to longevity-focused services. The Legacy tier covers unlimited visits, full testing, and complete primary care with longevity medicine.

Aevitas does not accept insurance for membership fees. The practice operates outside the insurance system, which allows longer appointments and a more personalized approach to care. Members may use HSA funds to cover membership costs. Standard health insurance still covers emergency care and other services outside the practice.

Gift cards for select services are also available.

Benchmarking at Aevitas gives patients a comprehensive baseline of their key health metrics. It includes a body composition DEXA scan, advanced lab testing with lipid and hormone panels, VO2 max testing, basal metabolic rate testing, a strength and movement assessment using the Vitruvian resistance platform, and a force plate jump and balance test. Dr. Valice reviews all results in a one-on-one session and provides a custom-printed booklet with personalized recommendations.

The Executive Health Physical is a full-day, personalized health and fitness assessment with Dr. Valice and an exercise physiologist. It uses comprehensive diagnostic testing and biomarker analysis to determine biological age and identify areas for optimization. The clinic closes to other patients during the visit for a fully private experience. Dr. Valice delivers a 12-month plan to support reverse aging and health optimization across every category. The Executive Physical is priced at $5,000.

Yes. Aevitas offers telemedicine visits for members and select services. This makes the practice accessible to patients who travel frequently or live outside the Traverse City area. Dr. Valice works with both individual and corporate clients who may not always visit in person.
