Active Health offers two membership tiers. The Essential Membership costs $170 per month and includes unlimited primary care, unlimited health coaching and nutrition sessions, all fitness classes, VO2 Max testing, a fitness tracker, and facility access. The Premium Membership costs $5,000 per year and adds physician continuity, specialist scheduling, 24/7 virtual consultations, home visits, and on-site emergency department consultations.

Yes. Active Health bills all medical visits through insurance. The membership fee covers enhanced access and integrated services beyond what insurance covers. Members should carry active insurance to use clinical services.

Active Health currently accepts new patients, but each physician caps their panel at one-fifth the industry standard. Spots are limited. Prospective patients can join the waitlist or submit an interest form through the Active Health website.

Yes. Premium Membership includes 24/7 virtual consultation access. Members can connect with a physician outside of regular office hours for urgent concerns. Essential Membership patients can contact the practice by phone during office hours.

Yes. Premium Membership includes home visits and short-term rehabilitation facility visits. The practice also provides on-site emergency department consultations for Premium members.

The Essential Membership includes unlimited fitness classes, access to the open-floor facility, a complimentary MyZone fitness tracker, free clinical-grade VO2 Max testing, unlimited health coaching and nutrition sessions, online personalized fitness programming, and up to 15% off personal training. Members also receive 15% off Fullscript supplements.

The 6 Point Promise is Active Health's medical standard of care commitment. It guarantees patients see their own provider at every visit, extended one-on-one time per appointment, timely scheduling, follow-up calls after visits, and a real person answering the phone. It also reflects the practice's limited panel size policy.

Each Active Health physician caps their patient panel at one-fifth the industry standard. This limit keeps care thorough and ensures same-day and timely appointments remain available. The practice may open a waitlist once panels reach capacity.

Active Health operates four locations in Connecticut. The primary care and fitness hub sits at 125 New Milford Tpk in New Preston. A second fitness location operates at 39 Park Lane Road in New Milford. The Farmington and West Hartford location serves the greater Hartford area. A fourth location in Wallingford offers fitness and health coaching services.

VO2 Max testing measures how efficiently the body uses oxygen during exercise. Active Health uses clinical-grade KORR technology for this assessment. The test takes about 10 to 15 minutes of active effort and 30 minutes total. Essential and Premium members receive the test at no additional cost. Non-members can book a session for $200.
