She founded the practice in 1999 after completing her Geriatrics fellowship at UC San Diego, and she has called Palm Desert home ever since.

The practice serves temporarily and permanently homebound patients who find office visits difficult or impractical. Dr. Mann schedules visits within 48 hours of a request. She can serve as a patient's primary care physician or work alongside existing providers, with no referral required. The practice brings diagnostic technology into the home, including laboratory testing, mobile X-rays, EKGs, echocardiograms, and ultrasounds. Dr. Mann also coordinates care with visiting specialists in podiatry, dentistry, optometry, physical therapy, and hospice. A Post Hospitalization Program helps reduce hospital readmissions, and 24/7 phone triage support keeps patients connected around the clock.
