Membership costs $235 per month or $2,820 per year. A one-time registration fee of $100 covers gathering and reviewing your records before your first visit. Your membership covers unlimited office visits, extended appointment times, same-day or next-day scheduling, sick visits, wellness visits, urgent visits, and hospital follow-ups. It also includes direct communication with your doctor by text, call, or email, routine labs, Pap smears, EKGs, and help coordinating specialist referrals.

Upper Valley Direct Primary Care does not bill insurance or Medicare. Your monthly membership fee covers primary care through the practice. If you carry insurance or Medicare, those benefits still apply to services outside the practice, such as specialist visits, hospitalizations, labs, and imaging.

When you join the practice, your doctor gives you a personal cell phone number. You can reach them by text, call, or email. The practice also offers telehealth visits by phone or video when an in-person visit is not necessary.

The practice does not prescribe long-term opioid medications for chronic pain. Both physicians will work with patients on a multifactorial plan to reduce pain and improve function. This typically includes referrals to physical therapy, behavioral health, alternative medicine providers, and pain specialists alongside primary care visits.

The practice asks that you provide written notice of cancellation. Your membership continues for 30 days after your final payment, giving both you and the practice time to transition your care to a new primary care physician.
