Unify Care offers two membership tiers. The Unify Standard plan runs $499 per month or $399 per month with an annual subscription. The Unify Premium plan runs $999 per month or $799 per month with an annual subscription. Family plans are available upon inquiry. Patients who prefer not to commit to a membership can use the pay-per-visit option, which costs $399 for an in-home call or $149 for a telehealth visit.

Members also receive direct phone, text, and email access to their physician and wholesale medication pricing.

The Premium plan includes everything in the Standard plan plus expanded access. Members receive up to ten house calls per year and 24/7 unlimited telehealth. The plan covers in-home laboratory and select diagnostic tests, in-home echocardiogram and spirometry, and hospital or nursing facility visits with care coordination. Premium members also receive all routine vaccinations and injectables, longevity health screening, biomarker analysis across 100-plus markers, and anti-aging services at cost.

Yes. Unify Care accepts FSA and HSA payments for membership fees. Insurance reimbursement for membership varies, so the practice recommends checking with your insurer directly.

Yes. Dr. Kate Mower leads the aesthetics program. The practice offers Botox, dermal fillers, Sculptra, microneedling, and chemical peels both at the Redlands clinic and through in-home concierge visits. Botox is priced at $10 per unit, fillers at $700 per syringe, and Sculptra at $800 per vial or $2,800 for four vials. Standard members receive a 20% discount on aesthetic services. Premium members receive anti-aging treatments at cost.
