The 12-Month Ongoing Care Membership costs $5,000 and includes unlimited medical communication, periodic physician visits, lifestyle coaching, personalized meal plans, free body composition and thyroid ultrasound, and access to discounted labs and supplements.

The practice does not participate with any insurance plans for consultations or membership fees. Patients can still use their insurance for labs, imaging, and prescriptions ordered during their care. This direct-pay structure allows Dr. Makhija to offer extended visits without insurance restrictions.

Yes. Unified Endocrine & Diabetes Care sees patients in person at two locations: 7750 N. Fresno Street in Fresno and 911 Moraga Road in Lafayette in the Bay Area. Dr. Makhija also offers telehealth appointments for all California residents, regardless of location.

No referral is required. The direct-care model gives patients the ability to schedule directly with Dr. Makhija without navigating insurance-based referral requirements.

Yes. Members receive free diabetes education visits with Certified Diabetes Education Specialist and NP Edith Boldt. These visits complement Dr. Makhija's clinical care and cover glucose management, device training, nutrition, and lifestyle strategies.

Yes. Dr. Makhija supports FreeStyle Libre, Dexcom G6, and Dexcom G7 for continuous glucose monitoring. She also provides insulin pump training and management for Tandem and Omnipod devices. The practice offers remote monitoring of blood sugar and sensor readings as part of ongoing care.

Yes. Telehealth is available to all California residents for ongoing care, not just the initial consultation. The membership includes periodic physician visits, which Dr. Makhija can conduct virtually for patients who prefer or require remote access.
