Trillium Concierge Medicine charges $3,500 per year, or $295 per month, for an individual membership. This fee covers the primary care relationship with your physician. The practice does not charge co-pays per visit and does not bill insurance. Founding member pricing locks in the lowest rate the practice will ever offer.

Yes. Trillium encourages all members to maintain health insurance alongside their membership. The annual fee covers your primary care relationship, but insurance remains important for specialist visits, hospital stays, imaging, emergency services, and other care outside the practice.

Trillium guarantees next business day appointments for all members. For many concerns, the practice also offers same-day access. Members can reach their physician directly by phone, secure message, or video without going through a call center or triage nurse.

Yes. Trillium cares for patients from newborns through end of life, with no age restrictions. Both Dr. Casey and Dr. Nelson practice full-spectrum family medicine. The practice welcomes entire families under one membership structure, with plans available for individuals, spouses or partners, young adults, and children.

Dr. Casey and Dr. Nelson cover for each other when one is unavailable. Both physicians maintain familiarity with every member's chart, so you always have access to a physician who knows your history. You never deal with an unfamiliar on-call provider.

Yes. Members can connect with their physician by phone or video visit in addition to in-person appointments. Direct secure messaging is also available. This access goes straight to your physician, not a call center.

Trillium handles a wide range of care in the office, including lab draws, IV fluids, antibiotic injections, skin biopsies, IUD insertion, Nexplanon placement, colposcopies, and imaging coordination. The practice also offers tongue-tie diagnosis and scissor frenotomy, vaccinations, and prenatal lactation consultations.

Yes. Both Dr. Casey and Dr. Nelson hold MSCP (Menopause Society Certified Practitioner) certification. The practice provides comprehensive perimenopause and menopause care, including hormone replacement therapy. This is a clinical focus area for both physicians.

Yes. Trillium offers gender-affirming primary care and hormone management for transgender and non-binary patients. This care falls within the broader clinical scope of Dr. Casey and the practice as a whole.

Prospective patients can schedule a free 20-minute meet and greet with the Trillium team to ask questions and determine fit. No payment is required for this initial conversation. From there, the team walks new members through the enrollment process and helps identify the right membership plan.
