Dr. Ponnuru helps members choose specialists and coordinates ongoing care. He holds staff privileges at Menorah Medical Center and sees his own patients there during hospitalizations. If hospitalization occurs elsewhere, he coordinates with the treating physician. Tier 2, 3, and 4 members can add hospitalization coverage at Menorah Medical Center for a flat rate of $250 per hospitalization.

Members may cancel at any time with 30 days of written notice. If neither has occurred, a prorated refund may be available. Rejoining after cancellation requires approval and depends on panel availability.
