Total Primary Care

Total Primary Care structures membership fees by age tier. Tier 3 (age 40 or younger) starts at $1,000 per year per person. Tier 2 (ages 41-54) is $1,400 per year. Tier 1 (ages 55 and older) is $2,200 per year. A Tier 4 option at $600 per year is available for dependents ages 16-26 who have a parent or guardian enrolled. Joint rates for spouses and partners are also available. Every tier includes a $20 fee per office or telehealth visit, due at the time of service.

Total Primary Care does not accept insurance, Medicare, or Medicaid for membership fees. The practice does keep insurance information on file to submit for outside services such as specialist visits, advanced imaging, and medications. The practice strongly encourages members to carry insurance for those non-covered services.

Yes. Total Primary Care accepts HSA payments for membership fees.

Members pay annual fees on a quarterly, semi-annual, or annual schedule by check or credit card. Membership begins on the date the signed contract arrives at the practice and renews annually on that date.

All membership tiers include an annual comprehensive physical with a review of medical records and history, comprehensive lab work, and a personalized plan of care. Tier 1 members also receive an EKG, yearly flu shot, and full hospitalization care at Menorah Medical Center. Tiers 2, 3, and 4 can add those services for a small additional fee. Discounted pricing on select medications is available to all members.

Yes. The practice frequently has same-day availability for office visits. Dr. Ponnuru caps his panel at 600 patients to maintain that level of access for members.

Yes. Total Primary Care offers telehealth visits for all membership tiers. The standard $20 visit fee applies to telehealth appointments, due at the time of service.

Total Primary Care offers several specialty wellness services. Emsella treats urinary incontinence using non-invasive pelvic floor technology. Emtone addresses cellulite using combined thermal and mechanical energy. TED Hair Restoration targets hair loss. The Galleri test by Grail screens for more than 50 types of cancer from a single blood draw.

Dr. Ponnuru helps members choose specialists and coordinates ongoing care. He holds staff privileges at Menorah Medical Center and sees his own patients there during hospitalizations. If hospitalization occurs elsewhere, he coordinates with the treating physician. Tier 2, 3, and 4 members can add hospitalization coverage at Menorah Medical Center for a flat rate of $250 per hospitalization.

Members may cancel at any time with 30 days of written notice. If neither has occurred, a prorated refund may be available. Rejoining after cancellation requires approval and depends on panel availability.
