About

At RiverRock Medical, Dr. Zain Hakeem delivers a form of concierge medicine built on a single, unconventional premise: health is autonomy. He defines it as the power to choose which dreams you want to make real. That philosophy shapes every clinical decision at the practice.

Dr. Hakeem caps his panel at 50 paying patients, plus 10 pro-bono spots, to maintain what he calls ICU-level focus. The practice centers on MeaningSpan, his framework combining longevity optimization with meaning, purpose, and connection across ten life categories. RiverRock offers two main entry points: the Cornerstone Assessment, a six-hour intensive covering physiology, cognition, and identity, and the Annual Reserve, a $25,000 membership available by invitation to Cornerstone alumni. Dr. Hakeem built his clinical approach on Bayesian epistemology, and he created Rx-Bayes, a decision support tool that makes probabilistic reasoning explicit in practice. Services range from cardiopulmonary exercise testing and metabolic mapping to EEG neurofeedback, osteopathic manipulation, medical hypnosis, and ultrasound-guided nerve blocks.

Frequently Asked Questions

The path to membership starts with the Cornerstone Assessment, a six-hour intensive covering physiology, cognition, and identity. Dr. Hakeem extends Annual Reserve invitations exclusively to alumni of that assessment. Prospective patients can also book an Alignment Audit, a one-hour consultation with Dr. Hakeem, to determine whether the Cornerstone Assessment is a good fit. RiverRock also hosts a free live webinar, An Evening With Dr. Z, as a starting point for anyone curious about the practice's approach.

The Annual Reserve costs $25,000 per year. RiverRock extends invitations only to patients who have completed the Cornerstone Assessment. The practice limits the panel to 50 paying members per physician to maintain intensive, focused care. The Cornerstone Assessment itself costs $5,500 and serves as the required prerequisite.

RiverRock does not accept any insurance. HSA funds cannot apply to RiverRock membership fees. Dr. Hakeem works exclusively for his patients, with no third-party obligations shaping clinical decisions.

The Cornerstone Assessment is a six-hour intensive day covering three areas: physiology, cognition, and identity. On the physiology side, it includes a full executive physical, VO2 Max testing, metabolic and mitochondrial analysis, spirometry, echocardiogram, EKG, bloodwork, strength assessment, and a 10-year cardiac risk evaluation. Cognitive testing includes a baseline function assessment and a neurofeedback evaluation. The identity portion covers values, personality inventory, meaning categories, and purpose analysis. A follow-up optimization planning session rounds out the assessment.

MeaningSpan is Dr. Hakeem's framework for combining longevity with depth and purpose in life. It organizes meaning across ten categories, including intimacy, family, community, personal growth, self-expression, impact and legacy, and spirituality. RiverRock treats MeaningSpan as a clinical target, not a motivational concept. The Cornerstone Assessment maps a patient's full MeaningSpan as part of its evaluation.

RiverRock focuses on five longevity factors with the strongest outcome data: VO2 Max, muscular strength, social connectedness, smoking status, and blood pressure. Dr. Hakeem applies Bayesian reasoning to prioritize interventions with real outcome evidence. The practice explicitly rejects aggressive biomarker optimization and supplement protocols that lack meaningful outcome data.

Yes, RiverRock offers clinical telehealth, including a home kit for Annual Reserve members. Dr. Hakeem also conducts telemedicine visits as part of ongoing care. Out-of-state clients can arrange travel visits with Dr. Hakeem for an additional investment.

Dr. Hakeem created Rx-Bayes as a clinical decision support tool that calculates real-time probability adjustments based on test characteristics and pre-test probability. It makes Bayesian reasoning explicit and reproducible in clinical practice. Patients benefit from this framework during test interpretation, diagnostic workups, and treatment decisions throughout their care at RiverRock.

RiverRock does not co-manage care with patients who prioritize podcast recommendations over physician judgment. The practice also does not prescribe on demand for patients seeking a provider to validate self-directed protocols. Dr. Hakeem practices medicine, not order fulfillment. Patients who want a collaborative, evidence-grounded clinical partnership are the right fit for this practice.

Yes. Dr. Hakeem reserves up to 10 pro-bono spots per physician for patients who cannot afford membership. Prospective patients interested in those spots can reach out directly to RiverRock to discuss eligibility.
