The Annual Reserve costs $25,000 per year. RiverRock extends invitations only to patients who have completed the Cornerstone Assessment. On the physiology side, it includes a full executive physical, VO2 Max testing, metabolic and mitochondrial analysis, spirometry, echocardiogram, EKG, bloodwork, strength assessment, and a 10-year cardiac risk evaluation. Cognitive testing includes a baseline function assessment and a neurofeedback evaluation. The identity portion covers values, personality inventory, meaning categories, and purpose analysis. A follow-up optimization planning session rounds out the assessment.

MeaningSpan is Dr. Hakeem's framework for combining longevity with depth and purpose in life. It organizes meaning across ten categories, including intimacy, family, community, personal growth, self-expression, impact and legacy, and spirituality. RiverRock treats MeaningSpan as a clinical target, not a motivational concept. The Cornerstone Assessment maps a patient's full MeaningSpan as part of its evaluation.

RiverRock focuses on five longevity factors with the strongest outcome data: VO2 Max, muscular strength, social connectedness, smoking status, and blood pressure. Dr. Hakeem applies Bayesian reasoning to prioritize interventions with real outcome evidence. The practice explicitly rejects aggressive biomarker optimization and supplement protocols that lack meaningful outcome data.
