The annual enrollment fee at Premier Concierge Medicine covers all primary care services Dr. Mazzone provides throughout the year. That includes same-day and next-day appointments, 24/7 cell phone and text access, the annual Executive Physical, lab work, x-rays, flu shot, ear lavage, minor suture repair, shave biopsies, cryotherapy for minor skin lesions, same-day prescription refills, specialist referral coordination, hospital care coordination, personalized nutrition and weight management counseling, a quarterly newsletter, and periodic wellness seminars.

The annual enrollment fee is $3,100 for an individual and $6,000 for a married couple. Members pay the full amount upon enrollment or spread payments across quarterly installments. The fee covers a one-year period. Premier Concierge Medicine accepts FSA and HSA funds toward the enrollment fee.

Yes. Enrolling at Premier Concierge Medicine does not replace health insurance. You still need coverage for specialist visits, hospitalizations, surgeries, diagnostic imaging, medications, and any care outside the scope of primary care. The enrollment fee covers only Dr. Mazzone's primary care services.

Premier Concierge Medicine does not bill health insurance for primary care services. The annual enrollment fee covers all primary care visits directly. Dr. Mazzone can provide an Explanation of Benefits that you may submit to your insurance company for potential reimbursement of specific services.

Yes, with conditions. If you discontinue enrollment before receiving your annual Executive Physical, Premier Concierge Medicine refunds the pro-rated portion of your fee based on unused months. If you pass away before your Executive Physical, your estate receives that same pro-rated refund. No refund applies after you have completed your Executive Physical.

No. Dr. Mazzone has opted out of Medicare, and the enrollment fee cannot be charged to or reimbursed by Medicare. He is also not a participating provider for CCPN. Patients who carry CCPN insurance would still need a separate Primary Care Physician for CCPN-required authorizations.

Premier Concierge Medicine hosts periodic free wellness seminars and community health events for enrolled patients. Events include healthy cooking classes, yoga and meditation sessions with specialist Ana Hernandez, and group hikes with Dr. Mazzone. These events are included as part of the membership.

Enrolled patients have direct access to Dr. Mazzone's personal cell phone for calls and texts, available 24 hours a day, seven days a week. m. m. Patients also manage health records and communications through the Healow patient portal.
