The Summit plan gives members direct access to Dr. Katof's personal cell phone, same-day or next-day appointments, and unhurried visits with no intermediaries. It also includes a comprehensive annual executive physical, preferred specialist referrals with expedited scheduling, free prescription delivery, and discounts at local partner merchants.

The Apex plan includes everything in the Summit plan plus home visits. Patients who want the option of in-home care from Dr. Katof choose this tier.

Yes. The Snowbird plan provides the same benefits as the Summit plan at a 20 percent discount. It suits patients who split their time between Long Island and another location for extended periods each year.

Yes. The Bespoke Healthcare plan serves adults ages 18 through 39 in three age-based pricing tiers. It mirrors the Summit plan and adds unlimited telehealth visits and coordination with student health services, local specialists, and pharmacies.

The practice accepts insurance for labs, imaging, and specialist visits. The monthly membership fee itself is separate from insurance, though many patients pay it with an HSA account.
