Pazona MD offers two monthly membership tiers. The Essentials plan costs $99 per month and includes up to two office visits per year with a nurse practitioner or PA, same-day visits, provider access via email, text, and portal, prescription management, a complimentary hormone panel, a complimentary GLP-1 trial, and a 20% discount on services. The Select plan costs $149 per month and adds one telemedicine visit, an annual comprehensive in-person wellness check for four total visits per year, free quarterly blood draws, and a 25% discount. Both plans require a minimum three-month commitment.

Yes. Pazona MD offers a la carte pricing for patients who prefer not to join a membership. A new patient consult runs $499 for a 60-minute visit. Follow-up in-person consults are $249 for 30 minutes, and follow-up telemedicine consults are $199. Uro/gyn procedures are $150 plus the cost of any medications or devices.

Pazona MD does not accept insurance for vasectomies or membership fees. The practice opted out of insurance for vasectomies specifically to offer clear, lower pricing and remove the administrative middleman. Patients pay directly, which keeps costs transparent.

The PMD Vasectomy uses a no-needle, no-scalpel, no-clips technique that Dr. Pazona developed. The procedure takes under ten minutes to complete. Pazona MD offers same-day consultation and procedure scheduling so patients can handle both in one visit. Most patients return to normal activity within three to seven days.

Yes. Pazona MD offers telemedicine visits on Fridays. Members on the Select plan receive one additional telemedicine visit per year as part of their membership. Follow-up telemedicine consults are available a la carte for $199.

Pazona MD offers a full range of women's urological and sexual health services. These include female incontinence treatment with Solyx and Axonics therapy, pelvic organ prolapse treatment, interstitial cystitis care, vaginal atrophy and dryness treatment, and female sexual dysfunction treatment using MonaLisa Touch, Morpheus8V, FormaV, and VTone. The practice also provides BioTE bioidentical hormone pellet therapy and pelvic floor therapy.

Yes. Pazona MD runs a lifestyle and weight loss optimization program using GLP-1 agonist medications including semaglutide, Ozempic, Wegovy, and Mounjaro. Members receive a complimentary GLP-1 trial as part of their Essentials or Select membership.
