Membership at On Point Primary Care costs $85 per month for patients age 27 and older. Patients ages 0-26 pay $60 per month. A one-time, non-refundable enrollment fee of $95 applies per patient at sign-up, in addition to the first month's subscription. Families with more than three members pay a maximum of $190 in enrollment fees.

The subscription covers the vast majority of primary care services with no copays or additional fees for most visits. It includes unlimited clinic visits, annual wellness assessments, school and sports physicals, acute and chronic disease management, in-office lab tests, and many procedures such as laceration repair, joint injections, and skin biopsies. Roughly 90% of typical primary care needs fall within the subscription.

Yes. On Point Primary Care accommodates same-day and next-day appointments during business hours for subscribers. The practice operates Monday through Thursday, 9:30 to 4:30, by appointment only. It does not operate as a walk-in clinic.

Subscribers reach Dr. Thomas 24 hours a day, 7 days a week through a secure patient portal via Elation Passport and encrypted text messaging through Hale Health. This direct line allows Dr. Thomas to triage concerns and coordinate care at any time.

No. On Point Primary Care does not contract with private or public insurance plans and does not bill insurance for services. The practice encourages subscribers to carry a high-deductible health plan or participate in a cost-sharing program, such as Liberty Direct or Samaritan Ministries, to cover hospitalizations or major medical events.

Yes. Dr. Thomas offers telemedicine visits for subscribers. The secure patient portal and text messaging system also support ongoing communication and care coordination outside of in-person appointments.

The practice orders common lab tests through LabCorp at 75-80% discounts for subscribers. Many in-office tests, including urinalysis, rapid strep, and urine pregnancy tests, come included in the subscription. Dr. Thomas also stocks commonly prescribed medications on-site at savings of up to 90-96% on some medications.

Subscribers at On Point Primary Care receive significant cash discounts on local outpatient radiology services. This covers MRI, mammography, DEXA scans, CT, ultrasound, x-ray, and PET/CT. Dr. Thomas coordinates these referrals directly on behalf of subscribers.
