Valley PM&R: Initial Consultation $350 (60-90 min in-person); Monthly Membership $225/month (3-month minimum, up to 2 follow-up visits/month, telemedicine or in-person, team conferences, care coordination, portal/text/phone access, priority booking, prescription renewals, discounted labs/imaging/prescriptions); Pay-As-You-Go Follow-Up Visit $125 (up to 30 min); Additional services $200/hour billed in 15-min increments. A monthly membership costs $225 per month with a three-month minimum. That membership includes up to two follow-up visits per month, telemedicine or in-person, care coordination, portal and phone access, priority booking, and discounted labs and imaging. Pay-as-you-go follow-up visits run $125 each.

Valley PM&R does not accept Medicare, Medicaid, or private insurance. Patients with private insurance can request a Superbill for potential out-of-network reimbursement. The therapy practices, Neuro Outlook and Function First OT, accept Medicare only and do not accept Medicaid or private insurance. Workers' compensation services are also available; contact the practice for pricing.
FSA and HSA funds typically do not apply to Valley PM&R membership fees. However, patients can generally use FSA and HSA funds for fee-for-service visits. The practice recommends confirming eligibility with your FSA or HSA administrator.
