Mila Family Health

Mila Family Health prices memberships by age, not by medical complexity. Children and young adults 17 and under pay $29 per month, or $19 per month when enrolled with an adult from the same household. Adults 18 to 44 pay $59, adults 45 to 64 pay $89, and adults 65 to 99 pay $119. The practice does not charge anything until after your first appointment.

Mila Family Health does not accept any form of insurance for payment. The monthly membership covers primary care directly. If you carry insurance, the practice can help you use it wisely for specialist referrals or other covered services outside the membership.

Members receive same or next-day visits, chronic and acute disease management, and unlimited calls, texts, emails, and video chats with Dr. Berezovsky. The membership also covers a wide range of in-office procedures, including laceration repair, joint injections, cyst and abscess care, toenail removal, wart removal, biopsies, ultrasound, Pap smears, IUD insertion and removal, casting and splinting, and medication administration. Patients pay only for the raw materials a procedure requires, not for the procedure itself.

Yes. Members receive Dr. Berezovsky's personal phone number and can call or text him directly. He responds promptly during regular hours and makes himself available after hours and on weekends for urgent matters.

Mila Family Health collects labs in-office, which keeps costs well below what outside labs or hospitals typically charge. For imaging, the practice negotiates cash-pay rates with local imaging centers so members know the price upfront, without surprise bills.

Yes. Mila Family Health is not insurance and does not replace it. The membership covers primary care, but you should carry insurance for unexpected or catastrophic events. If you are uninsured, the practice still provides full primary care through the membership.

Yes. Dr. Berezovsky refers members to specialists when needed. If you carry insurance, the practice works with you to coordinate a referral. If you are uninsured, Dr. Berezovsky works to identify cash-pay specialty options.

Mila Family Health accepts new members on a first-come, first-serve basis. You can complete a brief enrollment form on the practice website or contact the office directly to schedule an initial visit or an informational session. The practice does not charge you anything until after that first appointment.
