Concierge Membership is $500/year or $300 per 6 months and includes all Enhanced benefits plus private direct physician contact 7 days/week, complimentary HealthTrax Elevate6 referral, 10% discount on Regenerative Medicine (up to $1000), no charge for medical forms, and 20% discount on supplements.
