Maine Street Medical charges $175 per month or $1,800 per year if paid upfront. Paying annually saves the equivalent of two months. A one-time $100 sign-on fee applies per member. The practice also offers family rates; call the office to discuss options.

Membership covers all preventive and acute visits, plus the practice's Lifestyle Medicine programming. Non-Medicare members pay no surprise copays for visits. Medicare patients pay their membership fee and the practice bills Medicare separately, so standard Medicare copays may apply.

It includes home visits for all well-child checks from birth through two months and a lactation specialist to support breastfeeding. Dr. Thyssen and Dr. Brouillette also welcome prenatal meet-and-greet visits with expectant parents before the baby arrives.

Medicare members receive a Lifestyle 360 Visit each year. That visit includes a full physical exam, EKG, spirometry, body composition analysis, vision screening, an advanced cardiac risk panel, and mental and cognitive health assessments. It also includes a 12-class pass to Mountain Path Wellness upstairs for tai chi or qigong. The practice also offers Chronic Care Management phone calls every one to two weeks and Remote Patient Monitoring for weight, blood pressure, and blood sugar.
