Membership at Lakes Region Direct Primary Care costs $160 per month for adults ages 18-39 and $200 per month for adults 40 and older. A one-time registration fee of $150 applies when joining. Yes. Dr. Conkling makes house calls within 15 miles of the practice for an additional fee of $125. She also visits patients in nursing homes. Patients beyond the 15-mile range should contact the practice to discuss availability.

A few services fall outside the monthly membership. House calls and nursing home visits cost $125. Cryotherapy, skin lesion excision, wound care, incision and drainage, and trigger point injections each cost $20 to $40 depending on complexity. Services such as immunizations, imaging, lab work, specialist visits, and emergency or hospital care are not covered by membership.
