Lake Avenue Wellness

Individual memberships start at $249 per month for adults not currently enrolled in another Lake Avenue Wellness program. Existing patients qualify for a discounted rate of $199 per month. Couples memberships run $399 per month for new patients and $349 per month for existing patients. A one-time registration fee of $50 per person applies at enrollment. Members who sign up for an annual membership receive one month free.

Membership covers unlimited in-person and telehealth consultations, same-day and next-day appointments, annual bloodwork, comprehensive lab panels, physical exams, vaccine administration, chronic disease management, and minor in-office procedures. Members also receive deep discounts on prescription medications and free next-day prescription delivery. There are no co-pays beyond the monthly fee.

Lake Avenue Wellness does not bill insurance for membership services. The DPC model replaces insurance-driven billing with a transparent monthly fee. Members may choose to maintain a separate insurance policy for specialty referrals or hospital care, but the practice handles the vast majority of primary care needs directly.

Yes. The practice offers same-day and next-day appointments for both sick visits and routine care. Lake Avenue Wellness keeps its patient roster smaller than traditional primary care practices, which allows the team to provide prompt access without long waits.

Yes. Members receive unlimited telehealth consultations as part of their membership. The practice also connects with patients by phone and video conference, giving flexible options for those who cannot come in person.

The Ageless Protocol™ is a proprietary wellness program offered by Lake Avenue Wellness. It integrates multiple services available at the practice, including hormone optimization, metabolic testing, and other functional medicine tools, into a structured plan designed to support long-term vitality and healthy aging.

Yes. Lake Avenue Wellness actively serves first responders, blue collar workers, and truck drivers, including with specialized testosterone replacement therapy programs tailored to the demands of those occupations. The practice designs care around the schedules and health needs of physically demanding careers.

The membership covers ongoing management of conditions including asthma, diabetes, elevated cholesterol, high blood pressure, hypertension, hyperlipidemia, migraine headaches, and thyroid disorders. Members work directly with their physician to build a management plan without the constraints of insurance-driven visit limits.

Yes. Beyond primary care, the practice offers hormone replacement therapy for men and women, BHRT, menopause treatment, erectile dysfunction treatment, medical weight loss, IV vitamin therapy, hyperbaric oxygen therapy, peptide therapy, ADHD treatment, acoustic wave therapy, metabolic testing, and functional medicine.
