The Institute for Medical Wellness has not participated with insurance plans since 2008. The practice does, however, use your insurance for most laboratory services and any care performed outside the office, according to your policy. Dr. Horvitz is considered an out-of-network physician. Many patients carry high-deductible plans and find their overall costs comparable or lower.

Yes. The practice accepts both Health Savings Accounts and Flexible Spending Accounts. Starting January 1, 2026, the 2025 One Big Beautiful Bill Act allows HSA holders to use pre-tax funds for membership fees at qualifying DPC practices like the IMW. Yes, Dr. Horvitz offers both telemedicine visits and house calls. Members can connect via the Spruce app for text and video communication, or through the Elation Patient Portal. House calls may carry an additional fee beyond the standard membership.

Appointments at the IMW run between 20 and 60 minutes. Dr. Horvitz does not overbook or double-book, so visits stay unhurried. The hallmark of membership is the Annual IMW Wellness Exam paired with Advanced Wellness Testing, which reviews biomarkers across metabolic, hormonal, nutritional, and immune systems.

The practice accepts members 12 years of age and older. This includes individuals and families, with family plans available for two adults and one or more children living at home between ages 12 and 24.

Dr. Horvitz offers a complimentary 15-minute Meet-N-Greet for prospective patients. This gives you a chance to ask questions and determine whether the practice fits your needs before committing. New patients pay a one-time registration fee upon joining.

Dr. Horvitz conducts most nutritional counseling himself and sells supplements directly through the practice. Members also have access to the Fullscript online dispensary. Supplement costs fall outside the membership fee.

The IMW Advanced Wellness Testing program evaluates biomarkers tied to cholesterol, heart disease, clotting risk, diabetes and pre-diabetes, liver and kidney and thyroid function, nutrient deficiencies, metabolic imbalances, and genetic risk factors. Dr. Horvitz reviews all results in detail with each patient. The testing informs an individualized plan focused on root causes rather than symptom management.

Yes. The practice offers a broad range of specialized testing including coronary calcium scoring, hormonal panels via blood, saliva, and urine, nutrient panels, gut testing such as SIBO, stool analysis, inflammatory markers, and microbiome testing through GI-MAP, and genetic evaluations through IntellxDNA. Labs used include Labcorp, Quest, Genova, Doctors Data, ZRT, DUTCH, and Diagnostics Solutions.
