The Connected Care Program layers subscription benefits on top of your existing insurance. Ikigai Physician Group offers three tiers: Essentials at $110 per month, Enhanced at $275 per month, and Comprehensive at $550 per month. Each tier adds services and benefits beyond standard insurance-covered visits.

The Essentials tier costs $110 per month or $1,100 per year. It includes convenience-oriented scheduling with appointments up to 60 minutes, prior authorization and insurance appeals assistance, scheduling follow-up for imaging and referrals, and a routine screening EKG.

Enhanced members receive everything in Essentials plus annual body composition testing with the Seca Medical Body Composition Analyzer Ultra. They also choose one annual benefit from options including home visit scheduling, a Medical Book of You health summary, hereditary cancer risk screening, a personalized travel medical kit, physician-led medication organization, or a routine annual physical exam. Comprehensive members receive all Enhanced benefits plus a second annual benefit choice, with additional options like cell-free DNA active cancer screening and more frequent body composition testing.

Yes, Ikigai Physician Group works with insurance for covered medical visits. The Connected Care subscription fees sit separately from insurance billing and cover the additional benefits and scheduling conveniences each tier provides. Patients without Medicaid or a contracted insurance plan can add a medical visit subscription for $100 per month.

Yes, home visit scheduling is available as a subscription benefit on the Enhanced and Comprehensive tiers. Patients on the Enhanced tier can schedule one home visit annually. Comprehensive members can schedule home visits up to every other month. The visit itself bills to insurance; the physician travel and scheduling convenience are part of the subscription.

The Medical Book of You is an annual health summary that your physician updates after your yearly visit. The practice prints it in a durable format you can share with caregivers, consulting specialists, or medical professionals when traveling. It is especially useful for patients managing multiple or complex medical conditions.

Yes. The Enhanced tier includes hereditary cancer risk screening through Myriad MyRisk, which identifies genetic risk factors for certain cancers. The Comprehensive tier adds access to cell-free DNA active cancer screening, such as the Galleri test, which screens for multiple cancers using a blood test before symptoms appear. Insurance does not currently cover the Galleri test.

The Ikigai physicians practice full-spectrum family medicine and care for patients from infancy through end of life. They see women, men, and non-binary patients across all age groups.

Ikigai Physician Group operates two clinics. The Fortuna clinic sits at 3429 Renner Dr, Suite B, Fortuna, CA. The Arcata clinic is at 3798 Janes Rd, Suite 15, Arcata, CA. Both locations are open Monday through Friday, 9am to 12pm and 1pm to 5pm.

Prospective patients can join the patient waitlist through the Ikigai Physician Group website. The practice recommends calling ahead before visiting either clinic, as the facilities do not admit visitors without a scheduled appointment.
