Hoffman Family Medicine, PLLC

Membership at Hoffman Family Medicine costs $200 per month. This fee requires payment before the first visit. The membership does not cover most lab fees, imaging, or specialist consultations. It does cover urine pregnancy tests, strep tests, and urinalysis performed in the office, as well as costs for testing and supplements individualized to each patient's needs.

The membership includes unlimited visits with Dr. Hoffman as both primary care physician and functional medicine doctor. It also includes unlimited visits with health coach Jennie Buffington LPN FMCHC, four nutrition visits per year with Kelly Martin DCN RDN CDN, and four physical fitness visits per year with physical therapist Chelsea Leahy PT DPT C-PS. Home visits within 20 miles of the office are included as well.

Hoffman Family Medicine does not accept insurance, Medicaid, or Medicare for membership fees. Patients pay Dr. Hoffman directly. Members can still use their insurance to cover labs, imaging, medications, referrals, and hospitalizations that Dr. Hoffman orders. Medicare patients may join by signing a waiver confirming that Medicare will not be billed for services.

Yes. Hoffman Family Medicine offers same or next day appointments based on patient need. Members contact the practice and the team determines the best way to address the concern quickly. Visits run 30 to 60 minutes depending on availability and the nature of the visit.

Yes. Hoffman Family Medicine offers video and phone visits in addition to in-person appointments. Dr. Hoffman and the team determine the most appropriate visit format based on the patient's situation.

Hoffman Family Medicine does not provide vaccines directly. Dr. Hoffman helps coordinate vaccine access through local pharmacies, the public health office, or other primary care providers.

Members may be able to use a Health Savings Account or Flexible Spending Account toward the membership fee, which covers medical office visits and services including an annual physical. The practice recommends consulting your plan administrator or tax accountant to confirm eligibility for your specific account.

No. Hoffman Family Medicine does not handle Workers Compensation or No-Fault cases.
