Yes. Family members living at the same address each receive 15% off their membership. Veterans, active-duty military, corrections officers, police officers, firefighters, and EMTs also qualify for a 15% discount. Employers can enroll their teams at group rates.

Yes. Dr. LaBuz manages anxiety and depression as part of primary care. He also provides lifestyle coaching, counseling, and tobacco and alcohol cessation support. These services fall within the standard membership at no additional per-visit charge.

The practice does not provide emergency or hospital care, recurrent controlled substance prescriptions, invasive procedures or biopsies, cosmetic procedures, home care, or labor and delivery services. Patients who need those services receive referrals and coordination support from Dr. LaBuz.
