Grace Direct Primary Care

Membership for an adult under 65 costs $90 per month or $990 per year. Adults 65 and older pay $130 per month or $1,430 per year. Couples under 65 pay $170 per month, and children of members under age 21 or in school pay $20 per month. A one-time initiation fee of $100 per head of household also applies.

The membership covers all primary care services, including yearly physicals with EKG and appropriate labs, well-child check-ups, sports physicals, pap smears, routine exams lasting 30 to 45 minutes, same-day or next-day sick visits, a yearly flu vaccine, in-office lab work such as CBC, metabolic panel, HbA1C, cholesterol, thyroid, PSA, and urinalysis, minor procedures like EKGs and ear wax removal, and after-hours access via phone, text, email, or video. After 25 visits in a year, a $20 office visit fee applies for each additional visit.

Grace Direct Primary Care does not accept insurance for membership fees or services. The practice recommends that members carry a wraparound insurance policy or health sharing plan to cover emergencies and specialist care. The practice does not participate in Medicare, and does not enroll Medicaid, Tricare Prime, or HMO plan members.
