Members pay an annual membership fee to Schwartz CCPHP, LLC, which administers the membership program and provides non-clinical enhancements. The fee does not cover professional medical services. Dr. Schwartz and the practice bill professional services separately to patients or their health insurance plans.

Yes. Gary D. C. participates with select health insurance plans, including Medicare. The practice accepts payment from those plans as payment in full for professional services, subject to applicable deductibles, copayments, and coinsurance. The membership fee itself is not covered by health insurance.

Members may submit the membership fee to a flexible spending account, health reimbursement account, or medical savings account at their own discretion. The practice makes no representation that any portion of the fee qualifies for reimbursement from those accounts.

Members may pay the annual membership fee in one annual payment, two bi-annual payments, quarterly, or monthly. Annual payments carry no processing fee. 5% fee.

Either party may terminate the membership agreement with 30 days written notice. If a member terminates early, the practice refunds the pro-rata portion of any prepaid membership fee, minus a $200 administrative fee.

Yes. The membership agreement renews automatically for successive one-year periods. Either party may prevent renewal by providing written notice at least 30 days before the end of the current term.

Schwartz CCPHP, LLC coordinates non-clinical enhancements designed to improve the overall healthcare experience for members. The full details of those enhancements appear in the Membership Handbook. The enhancements do not include clinical, diagnostic, or therapeutic services.

If Dr. Schwartz becomes unavailable for an extended period, Schwartz CCPHP, LLC may seek to identify a temporary replacement physician. If a replacement physician takes over, members retain the option to terminate the agreement and receive a pro-rata refund.
