Foundations MD offers two membership tiers. The Primary Care plan costs $125 per month. The Health Renewal plan costs $250 per month and adds quarterly VO2 max testing, body composition analysis, and personalized health optimization. Both plans carry a one-time $99 enrollment fee, which the practice waives with a 12-month commitment. Memberships qualify for HSA payment.

Foundations MD does not bill insurance for primary care services. Members pay a flat monthly fee directly to the practice. The practice recommends that members maintain a high-deductible or catastrophic insurance plan to cover hospitalizations, specialist visits, and surgeries outside the scope of primary care.

Yes. Members can use their existing insurance for labs, imaging, and specialist or hospital care. Foundations MD also offers wholesale lab pricing, transparent cash-pay imaging, and discounted medications through the practice's preferred partners.

Foundations MD offers same- or next-day availability for members whenever possible. Members also reach Dr. Edun directly by phone, text, or video for urgent concerns and follow-up questions.

The practice requires a 6-month initial commitment for all memberships. After those first six months, the membership converts to month-to-month and members may cancel at any time. A 12-month commitment waives the $99 enrollment fee.

Yes. Dr. Edun offers video visits for follow-ups, acute concerns, and routine care. Members access telemedicine through direct communication with the physician, alongside phone and text options.

The Health Renewal plan at $250 per month includes everything in the Primary Care tier plus quarterly VO2 max testing, body composition analysis, and physician-guided nutrition and exercise planning. Dr. Edun also integrates wearable health device data and tracks results longitudinally to refine each member's plan over time.

The Metabolic Assessment is a one-time $500 service for patients who want a comprehensive baseline without committing to a monthly membership. It includes resting and active VO2 max testing, body composition analysis, and a 60-minute physician consultation with a personalized nutrition and exercise plan. Many patients use it as a starting point before joining the Health Renewal tier.

New patients can schedule a complimentary introduction call to learn whether Foundations MD fits their needs. From there, they choose a membership tier and schedule their first visit. Dr. Edun then builds a personalized care plan around their health history, goals, and lifestyle.
