Foundation Primary Care contracts directly with patients at a monthly membership fee. The practice covers a broad range of family medicine services within that fee, including acute illness care, chronic disease management, preventive visits, phone consultations, and secure messaging. Patients pay no deductibles, co-insurance, or copays within the clinic.

Yes. Members keep their existing insurance for services the practice does not cover, including emergency care, hospitalizations, and specialty services. Dr. Taylor also provides cash price options for labs and tests so patients can compare costs and decide whether to use their insurance or self-pay.

Yes. Foundation Primary Care offers same-day and next-day appointments for members. Patients schedule in-person office visits or phone consultations through the patient portal or by calling the office. Non-urgent questions not requiring a visit can go through secure messaging in the patient portal.

OMT is a hands-on treatment Dr. Taylor uses to address areas of dysfunction in the muscles, joints, and soft tissue. She detects restricted motion or tenderness through a structural exam, then applies specific corrective forces to support the body's natural healing. Members pay $80 for up to 60 minutes of OMT. Non-members can also schedule OMT; new non-member patients pay $200 for a 90-minute session, and established non-member patients pay $100 for 60 minutes.

Yes. Dr. Taylor conducts phone consultations and communicates through secure online messaging. Patients use the patient portal to schedule phone visits, send messages, upload vitals like blood pressure and blood sugar readings, and access their medical reports and treatment plans.

Foundation Primary Care does not cover emergency services, hospital care, rehab, medical imaging, or medications. Dr. Taylor does not prescribe narcotic pain medications and does not perform DOT physicals, disability physicals, or workers' compensation services. The practice also does not communicate with insurance companies for authorizations, referrals, or verifications.

Current IRS rules remain unclear on this point, and legislation is still in flux. Foundation Primary Care recommends consulting your insurance or legal advisor to determine whether your HSA or FSA covers DPC membership fees.

Foundation Primary Care holds typical office hours Monday through Thursday from 7:30 AM to 5:00 PM and Friday from 7:30 to 11:30 AM. The office closes on federal holidays. Patients can find updated hours on the practice website and Google Maps. In-person visits require an appointment, and patients under 18 must have a parent or legal guardian present.

Prospective patients can register directly through the patient portal on the Foundation Primary Care website. You can also schedule a meeting with Dr. Taylor to ask questions before joining. The practice accepts new patients on a rolling basis.
