Elevate Health and Aesthetics

Adult membership starts at $5,000 per year for patients ages 27 and up. Young adults ages 18 through 26 pay $3,500 per year. Pediatric membership for children ages 0 through 17 costs $1,500 per year, and a parent must hold a practice membership to enroll a child. An Elite package is also available with expanded home and office visit benefits.

The standard membership includes unlimited office visits, telehealth, 24/7 direct access to Dr. Eller, an in-depth medical history review, an annual exam, functional and traditional lab interpretation, chronic and acute care management, referrals, and care coordination. All primary care visits fall under the annual rate.

The Elite package includes everything in the standard membership plus up to 12 home or office visits per year within 30 miles of the practice. It also includes four complimentary mobile phlebotomy lab draws per year and enhanced care coordination, including coordination with out-of-state primary care providers.

The practice does not bill insurance for office visits. Patients may use their insurance for labs, imaging, medications, and specialist referrals. For patients who prefer not to commit to a membership, the practice also offers hourly fee options at $350 per hour for medical visits.

Yes. Elevate Health and Aesthetics offers a complimentary 30-minute Meet and Greet for prospective patients. This gives you the opportunity to meet Dr. Eller, ask questions, and determine whether the practice is a good fit before enrolling.

Yes. Dr. Eller offers telehealth visits for members. The Elite package includes up to 12 home or office visits per year within a 30-mile radius of the practice, bringing care directly to patients who prefer to be seen at home or at their workplace.

Yes. Dr. Eller cares for patients from birth through older adulthood. Pediatric care covers ages 0 through 17, with a parent holding a concurrent membership. Young adult care covers ages 18 through 26. Adult membership covers patients ages 27 and up.

Standard appointments run 60 minutes. That length gives Dr. Eller time to review your history thoroughly, address multiple concerns, and build a meaningful care plan without feeling rushed.

Yes. Members have access to same-day appointments. Direct physician access and a smaller patient panel allow Dr. Eller to accommodate urgent needs quickly.

Yes. In addition to concierge primary care and functional medicine, the practice offers medical aesthetics including neuromodulators, chemical peels, and dermaplaning. Dr. Eller also provides osteopathic manipulative treatment, trigger point injections, hormone replacement therapy, weight optimization, preoperative medical clearance exams, vitamin shots, and functional lab testing.
