Domus Primary Care

Domus Primary Care offers two membership tiers. The Essential plan runs $150 per month and includes unlimited telehealth and discounted house calls at $375. The Signature plan runs $350 per month and includes everything in Essential, plus house calls at $200, white-glove care coordination, a dedicated patient advocate, direct physician access, preferred rates for all household members, quarterly house call check-ins, and a personalized Domus Medical Summary.

Domus Primary Care does not participate in any insurance networks, including Medicare. The practice bills services directly to the patient. Upon request, the practice provides superbills that patients may submit to their insurance carrier for potential out-of-network reimbursement. Patients who are Medicare-eligible or enrolled in Medicare cannot join the practice.

Yes. Both membership tiers include unlimited telehealth visits. Essential and Signature members access virtual care as part of their monthly membership. Non-members pay $75 per virtual visit.

Dr. Gagarkin conducts house calls at a patient's home, hotel, or office anywhere in Manhattan. He brings a full kit of portable diagnostic tools, including a POCUS ultrasound, 12-lead EKG, and point-of-care lab equipment. House calls run 45 to 75 minutes. Signature members pay $200 per house call, Essential members pay $375, and non-members pay $475.

Prospective patients can sign up directly through the practice's patient portal. If you schedule an individual house call before enrolling and then join within seven days, the practice applies your house call fee as a credit toward your first month of membership.

Yes. Dr. Gagarkin sees patients ages five and up. He provides pediatric care alongside adult preventive care, chronic disease management, and acute illness treatment.

Signature members receive white-glove care coordination through a dedicated patient advocate. This includes direct physician access, quarterly house call check-ins, preferred rates for all household members, and a Domus Medical Summary, which is a centralized record summary Dr. Gagarkin maintains for each Signature patient.

Yes. The practice offers same-day and next-day appointments. Members reach Dr. Gagarkin directly without phone trees or extended waits.

Yes. Dr. Gagarkin conducts visits in both English and Russian. He holds certification as a Russian Medical Interpreter through ALTA Language Services. The practice website is also available in Russian.
