Without a membership, a house call is $475 and a virtual visit is $125. ‍DOMUS members ($250/month) pay $250 per house call and $50 per virtual visit. Additional family members can also be seen during the same visit and are $200 each, or $100 for members.

No. Domus Primary Care does not participate in any insurance networks, including Medicare. The practice bills services directly to the patient. Upon request, the practice provides superbills that patients may submit to their insurance carrier for potential out-of-network reimbursement. Patients who are Medicare-eligible or enrolled in Medicare cannot join the practice.

Yes. Both membership tiers include unlimited telehealth visits. Essential and Signature members access virtual care as part of their monthly membership. Non-members pay $75 per virtual visit.

Dr. Gagarkin conducts house calls at a patient's home, hotel, or office anywhere in New York City. He brings a full kit of portable diagnostic tools, including a POCUS ultrasound, 12-lead EKG, and point-of-care lab equipment. House calls generally take about 45 to 60 minutes.

Prospective patients can sign up directly through the practice's patient portal. If you schedule an individual house call before enrolling and then join within seven days, the practice waives your membership fee for the first month.

Yes. Dr. Gagarkin sees patients ages five and up. He provides pediatric care alongside adult preventive care, chronic disease management, and acute illness treatment.

DOMUS Primary Care Membership includes direct physician access, priority scheduling with same-day and next-day availability, annual wellness visit, ongoing medication management, and care coordination with specialists and labs. Members also enjoy preferred rates: $250 house calls and $50 virtual visits (versus $475 and $125 without membership), and $100 per additional family member seen during the same visit (versus $200).

Yes. Dr. Gagarkin conducts visits in both English and Russian. He is a native Russian speaker and holds certification as a Russian Medical Interpreter through ALTA Language Services. The practice website is also available in Russian.
