The standard adult membership at Cottonwood Family Medicine runs $90 per month. Couples pay $155 per month, and a family of four pays $180 per month plus $25 per additional child. Members who pay annually receive a 10% discount.

The membership covers unlimited office visits, phone conferences, annual physicals, well woman exams, sports and school physicals, vision and hearing screenings, dermatology screenings, and in-house labs and procedures. Additional fees apply for select procedures and services. Members also access generic prescriptions at wholesale prices.

The practice partners with Idaho Counseling Collective to deliver Ketamine-Assisted Psychotherapy for trauma, PTSD, depression, and anxiety. Patients self-administer ketamine via nasal spray with oxytocin or sublingual troches under medical supervision. A doctor remains on-site during sessions. Preparation and integration sessions cost $200 per hour and often qualify for insurance. Ketamine sessions cost $150 per hour and run two to three hours on a self-pay basis.
