Cool Springs Direct Primary Care, PLLC opened in Brentwood in 2024. Blake Carden, PA-C, founded the clinic and serves as owner. He studied Sports Medicine and Kinesiology at Trevecca Nazarene University. He later completed a Master of Science in Medicine at Trevecca Nazarene University.

The practice works with one physician, collaborative physician Eitan Friedman, MD. Dr. Friedman practices internal medicine and is board-certified in interventional cardiology. Services include primary care, urgent care, chronic disease treatment, orthopedic care, and mental health management. Patients use in-person visits, telehealth, and direct messaging through the Spruce app. The Health Optimization Plan adds DEXA, RMR, VO2 max, biomarker labs, nutrition consultation, and exercise recommendations. Non-members can book one-time visits for $275.
