Chicago Direct Primary Care offers three plans. The Unlimited Care plan runs $150 per month with a $150 enrollment fee, waived if you pay annually. The Prevention Pro plan runs $250 per month with a $250 enrollment fee, also waived with annual payment. The All-in-One package is a one-time $975 fee covering a two-visit comprehensive assessment. Additional family members or partners join at reduced rates. The practice also offers a telemedicine-only plan at $99 per month for patients anywhere in Illinois or Indiana.

Yes. The practice requires a three-month minimum commitment for new patients. After that, you can cancel at any time with 30 days' notice. There are no long-term annual contracts.

Yes. Chicago Direct Primary Care offers a one-time non-member visit for $200. That covers up to 60 minutes with Dr. Raza and includes a free follow-up appointment to review any lab or imaging results.

Appointments at Chicago Direct Primary Care run up to 120 minutes. The practice offers same-day visits and direct access to Dr. Raza by call, text, or email. Telemedicine and house calls are also available.

The practice offers VO2 max testing, DEXA body composition scans, resting metabolic rate testing, continuous glucose monitoring, genetic testing, liquid biopsies, and whole body MRI. Dr. Raza uses these tools to build a detailed picture of each patient's metabolic and cardiovascular health. Testing takes place through the Blue Zone Longevity facility.

Yes. Dr. Raza leads a dedicated weight loss program available as an add-on to any membership plan. The program uses DEXA scans to track fat loss while preserving lean tissue. It incorporates semaglutide and tirzepatide when appropriate, continuous glucose monitoring, fitness tracker integration, personalized diet and exercise planning, and pre- and post-operative bariatric surgery care. Dr. Raza holds Diplomate status from the American Board of Obesity Medicine.

Chicago Direct Primary Care offers wholesale lab pricing to members. A complete blood count runs $5, a comprehensive metabolic panel costs $6, a lipid panel costs $7, and an HbA1c costs $7. The practice also maintains an in-house pharmacy stocked with generic medications at discounted prices.
