Bolanos Concierge Medicine offers three monthly membership tiers. 99 per month and includes same-day or next-day appointments, unlimited telemedicine, one office visit per month, after-hours physician access, travel medical assistance, pre-op evaluations, and specialty care coordination. 99 per month adds two office visits, appointments up to 45 minutes, home visits, hospital courtesy visits, and minor surgical procedures. 99 per month adds six annual PRP aesthetic sessions and six annual vitamin infusion sessions. The practice also offers a couples discount on memberships.

The practice does not bill insurance for membership fees or covered services. Members pay a single monthly fee that covers the services included in their plan. The practice strongly recommends that members maintain their own health insurance for services outside its scope, such as hospital stays, emergency care, prescriptions, and imaging.

You may submit your membership fee to a flexible spending account, health reimbursement account, or medical savings account. The practice does not guarantee reimbursement from these accounts. Check with your benefits administrator to confirm what your specific plan covers.

Yes. Dr. Bolanos offers 24/7 access by phone, text, and email to all members. After-hours physician access is included in every membership tier, starting with the Essential plan.

Yes. Bolanos Concierge Medicine includes unlimited telemedicine in every membership tier. Members can schedule virtual visits for a wide range of concerns without waiting for an in-person appointment.

Yes. Home visits are available to members on the Advanced and Premium plans. Hospital courtesy visits are also available at those tiers for members receiving inpatient care.

Yes. Dr. Bolanos holds USCIS civil surgeon approval and completes Form I-693 immigration medical exams at the practice. The exam includes a full physical evaluation, vaccination review, required screenings, and form completion. The practice offers weekend availability for immigration and DOT medical exams.

The practice offers medically supervised weight loss using Semaglutide and Tirzepatide. Dr. Bolanos starts with a thorough health assessment and creates a personalized treatment plan for each patient. She monitors progress regularly and adjusts dosages to support sustainable results.

Bolanos Concierge Medicine offers IV infusion therapy with 12 drip options, PRP therapy, PRF therapy, microneedling, and Botox. Premium members receive six annual PRP aesthetic sessions and six annual vitamin infusion sessions as part of their membership.
