The Annual Fee covers most primary care services Dr. Sirdar delivers in the office, including annual physicals with EKG and comprehensive lab testing, wellness consultations, preventive care counseling, chronic disease management, perimenopause and menopause management, mental health medication management for one psychiatric medication, pre-op evaluations, and medically-managed weight loss. It also covers same-day or next-day appointments, telemedicine visits, and home visits when clinically appropriate. Members receive direct cell phone access to Dr. Sirdar for priority or non-emergency matters.

Yes. Members can call Dr. Sirdar directly on her cell phone for priority or non-emergency matters outside of regular hours. For life-threatening emergencies, patients should go to the nearest emergency department. When Dr. Sirdar travels for conferences or vacation, a colleague covers the practice at no additional cost to members.

Dr. Sirdar coordinates specialist appointments directly on your behalf. She monitors your progress while you receive specialty care and reviews any outside medical records to maintain continuity. She has built collaborative relationships with leading specialists in the Johns Hopkins system, and she draws on that network to connect patients with trusted experts.

Yes. Dr. Sirdar holds telemedicine appointments Monday through Thursday from 9am to 4pm. Established patients can schedule telemedicine visits through the Elation Passport patient portal, which also provides secure messaging, lab result access, and health record review.

Yes, for Bethesda patients when travel is difficult following an acute or prolonged illness. Dr. Sirdar evaluates each request on a case-by-case basis. Home visits fall within the services covered under the Annual Fee.
