Membership Details
Annual Fee is $3,000 per year for one adult member, $2,500 for the spouse of an adult member, and $1,500 for each member aged 18-23 who is the child of an adult member patient. Three payment options: (1) in full upfront (discounted by $500 for adult member = $2,500); (2) two equal semi-annual payments; or (3) four equal quarterly installments ($750/quarter). There is a $500 one-time non-refundable registration fee. No co-pays for services covered under the Annual Fee. Dependents ages 18-25 can receive services on a fee-for-service basis.

For life-threatening emergencies, patients should go to the nearest emergency department. When Dr. Sirdar travels for conferences or vacation, a colleague covers the practice at no additional cost to members.
Dr. Sirdar coordinates specialist appointments directly on your behalf. She monitors your progress while you receive specialty care and reviews any outside medical records to maintain continuity.
