AzureWave offers three pathways. Patients choose limited traditional insurance-based primary care, an insurance-plus-concierge membership, or a concierge-only membership. Dr. Meier-Ham intentionally caps the total panel across all three options to preserve access and continuity.

The insurance-plus-concierge membership runs $1,950 annually. The concierge-only membership runs $2,400 annually. Patients who prefer traditional insurance-based care pay standard copays and deductibles through their plan.

Yes. AzureWave participates with Medicare Part B, select Medicare Advantage plans, Florida Blue, Aetna, Cigna, Humana, TRICARE Select, and UnitedHealthcare. Insurance participation is subject to change, so patients should confirm their plan's network status before scheduling.

Yes. AzureWave accepts HSA and FSA cards as well as major credit cards. Contact the office to confirm which specific expenses qualify under your plan.

The Superhuman Protocol integrates red light therapy, PEMF, and exercise with oxygen therapy in a single structured session. Dr. Meier-Ham supervises all advanced therapy sessions using the DaVinci Theralight 360 system. These therapies complement medical care and do not replace clinical evaluation or treatment.

No. AzureWave does not provide hospital-based care and does not admit or round at local hospitals. If a patient requires hospitalization, the hospital's inpatient team manages that care. For emergencies, patients should call 911 or go to the nearest emergency room.

New patients schedule appointments online through the Healow portal or by calling 321-518-0165. The practice asks all new patients to download and complete intake forms before their first visit. Completing paperwork in advance keeps the appointment focused on clinical care.
