At Arise Wellbeing, PLLC, Dr. Sara J. Martin delivers direct integrative family medicine to patients across Central Minnesota. Her practice draws on conventional medicine, nutrition, supplements, herbal therapies, and wellness coaching. She treats the whole person - body, mind, and spirit.
Dr. Martin trained at the University of Minnesota Medical School in Duluth and completed her family medicine residency in 2011. She later earned a fellowship through the Academy of Integrative and Holistic Medicine in 2021. Her own family's experience with medical emergencies and major life disruption shaped her approach to healing. Members who cancel must submit written notification. Arise Wellbeing will discontinue the membership upon receiving that notice. Any Fullscript supplement account will be archived. Dr. Martin will no longer prescribe medications after cancellation. If you want records forwarded to a new provider, you will need to submit a release of information following that provider's policies.
Yes, Arise Wellbeing welcomes returning members. A six-month waiting period applies from the date of cancellation. Re-enrollment requires a $400 re-enrollment fee plus the first month's fee. Monthly rates reflect current membership pricing at the time of re-enrollment.
Arise Wellbeing does not serve as an emergency room. For serious symptoms - chest pain, stroke symptoms, or possible fractures - Dr. Martin directs patients to the nearest ER immediately. However, members with access to Dr. Martin within one to two days can often avoid ER visits for issues like ear pain or sore throats. During a hospital stay or ER visit, members can message Dr. Martin and she can communicate with them as a patient advocate.
