Couples pay $2,000 each or $4,000 together annually. Children enroll at $75 per month. The practice also charges a $20 administration fee per visit.

Every membership includes the Executive Annual Physical at no additional cost. Members also receive 24/7 direct access to Dr. Miller by phone, text, video, and email. Scheduled visits run an hour or more, and same-day or next-day appointments are available.

Yes. Apex Concierge Health membership fees qualify for payment through most Flexible Spending Accounts and Health Savings Accounts. Using pre-tax dollars through these accounts can reduce your out-of-pocket costs by 30 to 40 percent.

The practice does not bill insurance for membership fees. Dr. Miller encourages members to keep their existing insurance for specialist visits, hospitalizations, and other services the practice does not cover. Medicare patients are welcome, but Medicare does not cover membership services; those patients sign a private relationship waiver.

Yes. Apex Concierge Health provides after-hours pediatric care and comprehensive primary pediatric care for patients 12 and older. Same-day pediatric family care has no age restriction and is available at limited or no additional cost to members.

The practice provides chronic disease management, mental health support for anxiety, depression, ADHD, and sleep disorders, weight loss management, EKGs, pre-operative evaluations, nutrition and exercise coaching, specialist referral coordination, and in-office procedures. Annual physicals are included in the membership at no additional charge.
