Altucare Primary Medical Care

Monthly membership rates at Altucare vary by age. Adults aged 19 to 39 pay $99 per month. Adults aged 40 to 69 pay $119 per month. Adults aged 70 to 99 pay $129 per month. Children aged 5 to 18 pay $59 per month when their parent holds a membership. Families cap their monthly total at $325 for two adults and children under 19. A one-time enrollment fee of $150 applies after the first visit. Young adult and student discounts are available through age 26 upon request.

Altucare does not bill health insurance for membership or visits. Patients pay the monthly membership fee directly. This structure removes co-pays, deductibles, and co-insurance from routine care. The practice does accept Medicare and Medicaid patients with a one-time signed waiver.

Altucare currently accepts new patients from a waitlist. Prospective patients can fill out the waitlist form on the practice website. Dr. Agricola also offers free Meet and Greet visits for anyone considering joining. The practice does not accept new patients aged 0 to 4 at this time.

Yes. Altucare provides 24/7 phone access for urgent matters. The practice also uses Spruce for secure messaging and Elation Passport for patient portal communication. Dr. Agricola holds in-person office hours Monday, Tuesday, Wednesday, and Friday. She is available for telemedicine appointments on Thursdays. General availability for messages and calls runs Monday through Friday, 9 AM to 5 PM.

Yes. me. She reserves Thursdays specifically for telemedicine appointments. Phone consultations are also available. These options work well for follow-ups, routine concerns, and after-hours urgent questions.

Yes. Dr. Agricola makes house calls for patients aged 65 and older, and at her discretion for others. Home visits start at $200 per visit, with pricing that varies based on clinical need and distance.

The monthly membership covers all office and telemedicine visits with Dr. Agricola, same-day and extended appointments, secure messaging, and after-hours phone access. In-office labs such as ECG, rapid strep, hemoglobin A1c, rapid flu, rapid COVID, urinalysis, fingerstick glucose, and urine pregnancy testing are included. Procedures like ear wax removal, skin tag removal, wart freezing, wound dressing, and nebulized bronchodilator treatment are also included. The practice charges no blood draw fees and no vaccine administration fees for members.

Yes. Altucare requires no long-term commitment, and members can cancel at any time. If a former member chooses to rejoin, the practice charges a $400 re-initiation fee per person. The practice also charges a $50 fee for missed appointments without cancellation.

Altucare cares for patients from age 5 through adulthood. Dr. Agricola holds board certification in both pediatrics and internal medicine, making her well suited for patients across a wide age range. The practice no longer accepts new patients aged 0 to 4.
