The annual fee covers most primary care services at A Squared Primary Care, including the comprehensive preventative health assessment and exam, five office or video visits per year, blood draws and urine collections, coordination of care, and enhanced access services. There are no co-pays for included services. Additional visits cost $150 each for in-office, $100 each for video, and $200 each for hospital visits beyond the included five.

Yes. The membership fee covers primary care services within the practice, but it does not replace health insurance. Members need existing coverage to pay for specialist visits, laboratory tests beyond those included, imaging, prescriptions, and hospitalizations.

A Squared Primary Care offers same-day or next-day regular appointments. For sick visits, the practice schedules patients within 24 hours on weekdays. Scheduled visits have a wait time of less than 15 minutes in the office.

Yes. Dr. Agrawal offers secure video visits through the FollowMyHealth patient portal, accessible on a computer, smartphone, or tablet. Each video visit counts as one of the five included annual visits. Additional video visits cost $100 each.

Dr. Agrawal coordinates directly with the hospitalist team as your personal physician. She handles phone consultations throughout your stay. If a hospital visit from Dr. Agrawal is clinically appropriate, it counts as two visits; additional hospital visits beyond your included five cost $200 each.

Yes. If you leave for any reason, A Squared Primary Care returns a pro-rata portion of the annual fee. If you have already completed your annual comprehensive preventative health assessment and exam for the year, $1,500 is deducted from that refund.

Contact the practice by phone or email to request an enrollment package. To become a member, you sign a Physician-Patient Agreement and submit the annual fee. The practice accepts patients in the order they join and stops accepting new members once it reaches capacity.

Yes, Medicare beneficiaries may join, but Medicare will not cover the annual fee or any services Dr. Agrawal charges separately. Members cannot submit claims to Medicare for her services. Dr. Agrawal can still write prescriptions and lab orders, and Medicare continues to cover all other providers and facilities.
